All House League and Provincial teams travelling off-Island are required to complete and submit a travel sanction request to RPEI, 30 days prior to departure.  

This form should be filled out by clubs to document any accidents which occur during sanctioned events. If either of the parties involved makes an insurance claim, the incident report form needs to be submitted as well.

Accident Claim Form

This form is to be filled out by anyone who needs to make an insurance claim. It should be submitted to RPEI within 15 days of the accident in order to make a claim.
